The Camden Coalition Health Information Exchange

Since 2010, the Camden Coalition HIE has provided South Jersey hospitals, health systems, and care providers real-time access to patients' medical information via a secure, web-based portal.

Since launching more than 15 years ago, the Camden Coalition Health Information Exchange (HIE) has served as a foundational tool for our regional care management and clinical redesign programs, as well as a data bridge between our partners’ work throughout South Jersey and beyond. Now with nearly 50 organizations contributing to the HIE database, it has become an invaluable resource for collaborative research, evidence-based practices, and improved outcomes in value-based care.

This fact sheet provides an overview of the history, mission, reach, capability, and benefit of the Camden Coalition’s HIE. It also includes examples of the HIE in action at the Coalition and throughout its statewide network of beneficiaries and contributors, as well as information about the accompanying referral system My Resource Pal.
