Putting Care at the Center 2026 is offering conference attendees the opportunity to participate in site visits to community programs, local organizations, and cultural highlights as part of the conference agenda. These site visits will showcase Oakland’s vibrant ecosystem of health and social care, and offer attendees the chance to forge new connections and enrich their conference experience beyond the walls of the hotel.

Site visits will take place on Thursday, October 15, between 3:30 – 6:00 pm and are included with your registration at no additional cost. Advance registration will be required, and capacity will be limited.  Site visit registration will open in August; additional details and updates will be shared as they become available

Programmatic site visit options

Alameda County Health Care for the Homeless

Alameda County Health Care for the Homeless (ACHCH) is part of the Housing and Homelessness Services (H&H) Department within the Alameda County Health agency. Since 1988, ACHCH has addressed the needs of Alameda County residents experiencing homelessness through direct care and a contracted network of community providers to deliver low-barrier, culturally responsive, and linguistically relevant homeless healthcare services. LifeLong Medical Care, a federally qualified health center FQHC, is one of ACHCH’s community provider partners.

This site visit will include a close look at ACHCH’s street medicine van, tour of Trust Health Center operated by Lifelong Medical Care, an overview of ACHCH’s diverse range of services, and an opportunity for Q&A.

Alameda Point Collaborative

Alameda Point Collaborative (APC) provides affordable housing and supportive services for formerly homeless individuals and families, including people with disabilities. APC’s community includes a teen center, youth center, workforce development programming, food pantry, access to an organic farm, plant nursery, and community garden, all aimed at supporting a holistic and thriving community. This site visit will include a tour of APC’s affordable housing community, a presentation on the organization’s programs and services, and an opportunity for Q&A with APC leadership.

Asian Health Services

Asian Health Services (AHS) provides medical, dental, and behavioral health services to more than 50,000 patients. Committed to whole patient health, AHS also addresses the social factors of health including food insecurity, nutrition, youth leadership, and culturally competent care to invest in its community’s overall health, support safer communities, patient empowerment, and foster vibrant communities.

This site visit will include a tour of Chenming & Margaret Hu Medical Center (HMC), a presentation on AHS’s work, and an opportunity for Q&A with staff.

Resources for Community Development (RCD)

Resources for Community Development (RCD) has created and preserved affordable housing across the Bay Area for over 40 years, supporting individuals and families with the fewest housing options. RCD now serves more than 6,150 residents across 25 cities through affordable housing development, resident services, community development, and asset management.

RCD also serves as the backbone organization for the Unincorporated Health and Wealth Initiative (UHWI), which supports residents in Alameda County’s unincorporated communities, including Ashland-Cherryland and the Eden Area. In partnership with Eden United Church of Christ and its nonprofit, the Eden Power Collective, UHWI works to strengthen resident leadership, expand health and wellness supports, and advance community-led advocacy in areas that often lack local city government and decision-making infrastructure.

This site visit will take place at Madrone Terrace, an RCD affordable housing community, and will include a tour of the complex, an overview of RCD and UHWI’s community development model, and an opportunity for group discussion and Q&A.

Roots Community Health

Founded in Oakland, California, Roots Community Health has the mission of uplifting those impacted by systemic inequities and poverty through medical and behavioral healthcare, health navigation, workforce enterprises, housing, outreach, and advocacy.

This site visit will offer an immersive tour of Roots Community Health’s Main Clinic Site in East Oakland, with a firsthand look at Roots’ comprehensive open access/walk-in services, highlighting its integrated approach to behavioral health, housing supports, and walk-in medical screenings. Attendees will explore Roots’ essential wellness programs, including the courtyard garden, food access initiatives, and vital hygiene services designed to support the whole person.

Street Level Health Project

Street Level Health Project has served as the “safety net of the safety net” in Oakland’s Fruitvale District since 2002, supporting over 10,000 low-income adults across East Oakland and Alameda County. The organization works to address barriers to primary and preventive care for immigrants, particularly undocumented and recently arrived community members through services including resource navigation, food pantry access and mutual aid, benefits enrollment, health screenings, health literacy support, mental health education, and wellness services.

This site visit will include a tour of Street Level Health Project’s community center and day labor stop, a presentation on the organization’s services and community-based model, and an opportunity for group discussion and Q&A.

Cultural site visit options

African American Museum and Library at Oakland

The African American Museum and Library at Oakland (AAMLO) is dedicated to the discovery, preservation, interpretation, and sharing of historical and cultural experiences of African Americans in California and the West for present and future generations.

AAMLO’s reference library consists of approximately 12,000 volumes by or about African Americans, and is a unique jewel for researchers, students, and anyone interested in African American History. It contains books on religion, the military, civil rights figures such as Martin Luther King, Jr. and Malcolm X, and the Black Panther Party. At AAMLO, visitors can access online databases and selected newspapers, censuses, and other sources, view a collection of 400 videos and DVDs on-site, see the current exhibition, take seeds from the seed library, and meet staff.
