The National Center for Complex Health and Social Needs recently released the Complex Care Startup Toolkit. This toolkit contains over 200 tools, templates, and guides to support new and developing complex care teams. The toolkit contains resources from the Camden CoalitionĀ and dozens of partner organizations to help new and growing programs with program design, program operations, data & process improvement, team & leadership development, community mapping & collaboration, and communication & growth of success.

Join this webinar to heaed from two toolkit contributors. Dr. Dave Moskowitz of Navvis Healthcare shared insights into an activity he developed to establish role clarity on his team. Shelly Virva of the Health Management Associates introduced a resource for medications for addiction treatment (MAT) providers. Additionally, staff and subject matter experts from the National Center were invited to demonstrate how to use the Complex Care Startup Toolkit and answer questions from new or developing complex care teams.
