This brief examines the hurdles our care teams faced in their attempts to collect patient-reported data through the Center for Disease Control’s (CDC) Healthy Days measures, and shares lessons learned from a rapid cycle Plan-Do-Study-Act (PDSA) project we implemented to improve data collection. Included in the brief are recommendations to improve measurement implementation in a way that fosters authentic and mutually beneficial relationships across teams and with the individuals served by the program.
