Our strategic plan for 2022-2025 is the result of a deep dive into what we do well, what we have accomplished, and how we could best contribute to improving the health and well-being of people with complex health and social needs.

It not only outlines our bold organization-wide goal to confront inequities and system failures by strengthening the ecosystems of care for 500 communities in Camden, across New Jersey, and around the country, but also charts our path to get there, including the framework by which we will measure our success.
