Putting Care at the Center is a yearly opportunity for complex care leaders and innovators to connect and share best practices — but what exactly is complex care? In our experience, there are thousands of care providers, advocates, researchers, leaders, and more who are already doing complex care across the country without having a name for it. This pre-conference session offers an opportunity for attendees who are new to the field or simply looking to get grounded, to start the conference with a shared understanding of key ideas, language, and approaches that shape complex care.
This 3-hour pre-conference session offers a high-level overview of the foundational principles and structural components of complex care. Designed for care team members, supervisors, program managers and administrators, and systems leaders who are new to the field or seeking to build or broaden their understanding. Topics will include person centered approaches, health-related social needs (HRSN), harm reduction, trauma-informed care, and the importance of authentic healing relationships. The session will highlight practical tools for reflective supervision and strategies to build trust with communities, elevate lived experience, and develop sustainable community engagement and leadership structures like advisory committees.
In addition to front-line practices, the session will touch on systems-level components like backbone organizations, governance models, and payment mechanisms such as 1115 waivers and value-based care. The role of data, continuous learning, and quality improvement will also be discussed.
Real-world examples and introductory tools will be shared throughout to support understanding and future exploration. This session is intended to lay the groundwork for deeper conversations and learning throughout the rest of the conference.

About the facilitators

Renee Murray
Renee is the Director of Training and Education at the Camden Coalition. In this role, she creates content and facilitates trainings on our COACH and RELATE models for health systems and community-based organizations both locally and across the country. Renee is a subject matter expert on the Camden Core Model and the COACH and RELATE frameworks, having spent the past six years leading, guiding, and mentoring the RNs, LPNs, and CHWs on the Coalition’s Care Management Team, who work with the most medically and socially complex patients throughout the city of Camden, New Jersey.

Gladys Antelo-Allen
Gladys Antelo-Allen
Gladys is Associate Director of Education and Training at the Camden Coalition. In this role, Gladys manages relationships for Student Hotspotting, technical assistance contracts, and other educational programs the Coalition offers. Previously, she worked as a Nurse Care Coordinator on the Camden Coalition’s care team, working with patients in the community. Gladys is a Licensed Practical Nurse by training and graduated from Lincoln Technical Institute in 2012.
