From a healthcare perspective, we’ve known for years that health disparities and social inequities exist and have always approached every child with an equity lens. But sometimes it’s really just been talk. I think now we’re ready for action — and the only way a healthcare organization can do that is with community partnerships. The Ecosystem Community Learning Collaborative launched in spring 2021 as a group of multi-organization collaboratives from five communities across the country.
Each ecosystem site received technical assistance and coaching from our National Center for Complex Health and Social Needs, and met regularly as a group to learn from each other. The learning collaborative ended in fall 2022.
Learning collaborative participants were also interviewed for our brief on rethinking the value of cross-sector collaboratives.

Learning collaborative communities
Allegheny County, PA
Ecosystem aim: Improve healthcare outcomes for individuals experiencing homelessness and reduce the cost of emergency department services.
Participating organizations:
Camden, NJ
Connect or reconnect patients to outpatient mental health and substance use treatment services following presentation to the emergency department through our Pledge to Connect initiative.

San Diego, CA
Ecosystem aim: Improve the emotional well-being of children with complex needs and their families.
Participating organizations:
Spartanburg, SC
Ecosystem aim: Reduce recidivism, improve health outcomes, and reduce unemployment among individuals leaving incarceration in their communities.
Participating organizations:
York County, PA
Ecosystem aim: Achieve health equity and increased well-being for populations experiencing substance use, mental health, chronic medical, and/or poverty resulting from or resulting in interactions with the criminal legal system.
