2025 was a year of extraordinary strain for health and social care systems across the country. As communities navigated funding cuts, policy shifts, and persistent fragmentation, people with complex health and social needs felt the impact most. While the challenges facing the field today are significant, we remain confident—not because the road ahead is easy, but because navigating complexity is where this organization, and our partners, have always done our best work.
Our 2025 annual report highlights regional demonstration work across South Jersey, including a behavioral health regional triage pilot connecting emergency departments and community providers through shared data, as well as expanded real-time access to behavioral health data through the Camden Coalition Health Information Exchange. We also continued our eviction prevention work through our Medical-Legal Partnership and, in our role as a Regional Health Hub, supported NJ Housing Support Providers in delivering new Medicaid-funded housing services. Over the life of our 2022–2025 Strategic Plan, we supported well over 300 communities nationwide in strengthening their ecosystems of care.
We also advanced rigorous evaluation efforts—from assessing the impact of the Medical-Legal Partnership and Camden County Correctional Facility’s Peer Support Reentry Program (PRSP), to using rapid A/B testing to improve Medicaid outreach. Our teams continued to invest in the field by launching the new Complex Care Certificate for Team Leaders, convening nearly 800 complex care practitioners at our national conference, advancing authentic community engagement through INSPIRE, and piloting our Ecosystem Alignment Tool in Maine.
