Camden Care Management meetings are a monthly forum for community members and professionals interested in transforming healthcare and improving the wellbeing of individuals with complex health and social needs. These meetings provide opportunities to network, participate in case conferencing, learn about programs serving Camden residents, and participate in training on topics relevant to serving complex patients.
The Camden Coalition has been hosting provider breakfast meetings since its conception in 2002. The original meetings were designed by Dr. Jeffrey Brenner, Founder of Camden Coalition, and Carol Wolff, Executive Director of Camden Area Health Education Center, as a way for primary care providers from across the city to meet and discuss how to improve care for Camden residents.
Today, the meetings are open to all Camden residents; each meeting features a presentation from a program serving Camden residents or training sessions. Camden Care Management Meetings are currently being conducted virtually. Contact Kerona Sharpe at [email protected] for more information.
