Due to the unprecedented global COVID-19 pandemic, our staff have had to very quickly adapt a high-touch care management model to a remote workflow. Our top priority has been to stay connected with our vulnerable patients and community residents, providing accurate information about both the novel coronavirus itself as well as its impact on the availability of services in the community.
These documents represent current protocols and scripts that our staff are using to connect with patients and Community Advisory Committee (CAC) members. We are sharing them in the hopes that other providers and organizations can adapt them for use in their communities. As the situation on the ground is shifting rapidly, we expect to be updating and adding to these documents over the coming weeks.
