Ecosystems of care work collectively and intentionally to better address the root causes of poor and inequitable health and well-being among a specific population or community. However, few tools exist to identify gaps and measure sustained improvement in ecosystems. How do teams and partners know if they are strengthening the ecosystem to achieve better outcomes, value, and equity? The Camden Coalition’s Ecosystem Assessment Tool helps participants and partners in an ecosystem of care identify current strengths and opportunities to improve how they work together to serve their communities. It is a maturity model that can be used to quantify impact of an inter-organizational program, initiative, or intervention across key domains of an ecosystem. By strengthening ecosystems of care, organizations and government agencies can meaningfully improve health equity in their communities.
In this webinar recording, presenters shared the assessment tool, described how the Camden Coalition has used it to measure our impact, and demonstrated how webinar attendees can apply it to their own programs.
