We use quantitative and qualitative cross-sector data to identify and understand the population we serve, assess needs, coordinate services across South Jersey, adapt best practices, and continuously improve our ecosystem’s care delivery.

Using data to identify and engage patients with complex health and social needs — sometimes called “healthcare hotspotting” — is the foundation of our work. From our early days combing through hospital claims data to our Camden Coalition Health Information Exchange, we connect data across institutions and sectors to better identify and serve individuals with unmet needs.
How to measure the impact of complex care programs and cross-sector partnerships is another key area of focus. We work with national organizations and researchers to identify and test measures for evaluation and quality improvement.
