2024 was a promising year, one where we made major strides toward our mission of advancing equitable ecosystems of care across our country. We continued to focus on three core activities: demonstrating what works, expanding evidence, and sharing expertise rooted in our experiences in both frontline complex care delivery and ecosystem development.

Our 2024 annual report spotlights regional demonstration projects like connecting ED patients to outpatient behavioral health and pregnancy care, statewide initiatives like supporting primary care providers across NJ in treating opioid use disorder, and care transformation in other communities across the country, including supporting the rollout of CalAIM in California.

At our annual conference, Putting Care at the Center 2024, we introduced a new Ecosystem Assessment Tool as well as a set of core competencies for complex care team leaders. The INSPIRE partnership that we lead released a roadmap for effective community engagement in healthcare.

In total, from January 2022-June 2024, we supported 234 communities across the US in strengthening their ecosystems of care. Read the report to learn more.
