Our 2021 annual report highlights our major initiatives over the last year as we’ve continued to strengthen our complex care ecosystem. Highlights from the year include:

• Training COVID Community Ambassadors to address misinformation about COVID 
• The launch of the Camden Coalition Learning Center, an online hub that will host virtual courses on our COACH framework for patient engagement, the complex care core competencies, and more
• Contributing to the curriculum development of the New Jersey Department of Health’s new Colette Lamothe-Galette – Community Health Worker Institute
• Working with local health systems and behavioral health providers to strengthen the connection between emergency departments and ongoing care for individuals with mental health and substance use disorders through our Pledge to Connect initiative 
• The release of  three new toolkits, and the launch of our Ecosystems Community Learning Collaborative
