A recently published paper by Camden Coalition staff A Community-Centered Approach to Strengthening Perinatal Care Connections, published in the Journal of Maternal and Child Health The paper is a descriptive analysis of the patient population involved in their prenatal care initiation pilot that ran from 2021-2023. The transition-of-care workflow used the Health Information Exchange to identify individuals using the EDs in early pregnancy without outpatient pregnancy care and proactively offered support in connecting to needed healthcare and supportive resources. It demonstrates the feasibility of such a workflow, highlights early pilot learnings, and discusses patient engagement barriers and successes. The publication demonstrates the importance of learning from innovative feasibility pilot programs and testing new models of strengthening our maternal health ecosystem.
