How are you working to build and strengthen the ecosystem of care in your community? We are looking for promising practices and approaches to showcase to a national audience in a webinar series spanning fall 2024-spring 2025.

The Camden Coalition defines “ecosystems of care” as interdependent networks of organizations and sectors within a community that work collectively to provide care and address the root causes of poor health among individuals with complex health and social needs. Our Ecosystems of Care Assessment Framework defines six essential domains of a strong ecosystem of care.

We are looking for webinar proposals that highlight approaches, programs, and partnerships that span organizations and sectors in a community and show promise in improving the functioning of the community’s ecosystem of care along one or more of the domains in the assessment framework.

Selected webinar proposals will be hosted by the Camden Coalition’s National Center for Complex Health and Social Needs, giving presenters the opportunity to share their work with a diverse, national audience from a wide range of roles and sectors working to transform care delivery.

Find more information, including details of the ecosystem domains, webinar selection criteria, and what to expect if you are selected, in our call for proposals document. Submissions are due October 2, 2024. The deadline has been extended and submissions are open until December 31, 2024.

This webinar series will take place on the first Thursday of the month (with the exception of January). Please make sure presenters are available on those dates.
