For over two decades, the Camden Coalition has worked to improve outcomes for people with complex health and social needs. One of the most important insights from this work is that while our healthcare system is rich in strong, point-in-time interventions with resilient and dedicated care teams, they often operate within weak ecosystems, a reality that diminishes the effectiveness of providers and care teams. Addressing this challenge requires a commitment to changing norms of practice and building truly collaborative, cross-sector infrastructure.

In this webinar, presenters explored one such ecosystem-building practice: complex care conferencing. By bringing together healthcare providers, human services agencies, and community-based partners to solve real cases in a structured, coordinated way, case conferencing helps bridge the gaps caused by service fragmentation and begins to strengthen the infrastructure needed to support individuals with complex needs.

Three case studies from across the country that are using case conferencing to foster collaboration and drive systems change discussed their strategies:

  • New Jersey: Complex case conferencing for behavioral health patients utilizing the emergency room
  • Vermont: Statewide, team-based care model
  • Oregon: Health Share of Oregon’s coordinated care approach
