The Camden Care Management Meeting is a monthly forum for community members and professionals invested in improving the well-being of individuals with complex health and social needs.

They are open to anyone that lives or works in Camden and the surrounding regions. Meetings typically highlight a community resource or program and allow time for discussion and community updates. They provide the opportunity to:

• Learn about programs serving residents of Camden and surrounding areas,
• Network with other providers and community members,
• Participate in case conferencing, and
• Receive training on topics relevant to serving patients with complex needs.
