Fragmented care systems are failing those who need them most and burning out the care teams that are trying to help. Despite decades of investment in health IT and collaboration efforts, consumers with complex health and social needs still fall through the cracks. In this brief, the Camden Coalition reflects on experiences with complex case conferencing in two different states.

Complex case conferencing is a promising practice that helps align providers across sectors to serve individuals with complex health and social needs in a more coordinated manner. With committed partners and careful planning, complex case conferencing can build a stronger ecosystem of care with the potential to improve patient outcomes, partner communication, and efficiency of care.
