Patients with frequent hospitalizations commonly experience a broad range of legal barriers that impede their ability to meet their basic needs and stay healthy. To better address the health-related social needs of community members involved in our complex care intervention, the Camden Coalition has partnered with Rutgers Law School to pilot a medical-legal partnership.

The medical-legal partnership model integrates attorneys into healthcare settings to resolve social needs that can undermine a patients’ health and well-being and can be better met through the legal system.

When participants in our intervention encounter potential legal issues, our care team works closely with a consulting lawyer to assess their legal needs and provide legal guidance to help navigate around complex issues. When necessary, the lawyer accompanies the care team to meet with patients and directly provides them legal services.

By addressing the legal concerns faced by many patients with complex needs, we can better equip them with the knowledge and resources they need to manage their chronic conditions and improve their overall wellbeing.
