Complex care essentials is a pre-conference experience that introduces the core principles of complex care while creating more space for dialogue, application, and peer learning. Designed for attendees who are new to the field or looking to get grounded in shared concepts and language, this session combines self-paced learning with interactive, in-person discussion.
Participants will receive early access to six 30-minute, pre-recorded modules created by Camden Coalition subject matter experts. These videos provide high-level foundation across key areas of complex care, including complex care delivery, community engagement, cross-sector partnerships, ecosystem leadership, data and measurement, and payment and policy. Attendees can watch the modules at their own pace prior to the conference, ensuring a shared baseline and allowing the in-person session to focus less on lecture and more on conversation.
The in-person session is designed as a rotating, small-group experience. Six discussion tables, one for each Essentials topic, will be facilitated by subject matter experts. Participants will engage in a series of timed discussion rounds and can choose to rotate among topics or stay with the one most relevant to their work. Conversations may include Q&A, real-world case examples, practical scenarios, and shared problem-solving with peers.
This format is intended to lay a strong foundation for the rest of the conference, equipping participants with shared context while creating meaningful opportunities to learn from experts and one another.

Camden Coalition
Renee is the Director of Training and Education at the Camden Coalition. In this role, she creates content and facilitates trainings on our COACH and RELATE models for health systems and community-based organizations both locally and across the country. Renee is a subject matter expert on the Camden Core Model and the COACH and RELATE frameworks, having spent the past six years leading, guiding, and mentoring the RNs, LPNs, and CHWs on the Coalition’s Care Management Team, who work with the most medically and socially complex patients throughout the city of Camden, New Jersey.
