This credentialing application is designed for housing services organizations seeking to enroll as Housing Supports providers in Managed Care Organization (MCOs) networks. Please refer to Housing Supports Provider Guidance packet for submission information.

Housing Supports Initial Assessment Tool
The purpose of this form is to request Housing Supports services and document eligibility. The form can be completed by a NJ FamilyCare member, a parent, caregiver, family member, guardian, trusted friend or staff from an organization that helps a NJ FamilyCare member.
Housing Supports Level of Need Assessment Tool
The purpose of this form is for a Housing Supports Program Pre-Tenancy and Tenancy-Sustaining provider who is authorized to deliver services to a member to:
- Document eligibility for Pre-Tenancy and Tenancy Sustaining services.
- Request MCO authorization and reauthorization for continued Pre-Tenancy and Tenancy Sustaining services.
- Classify individuals into two risk categories (Lower Level of Need and Higher Level of Need) to inform service provision and reimbursement under the state’s 1115 Medicaid waiver program.
Housing Supports Housing Stabilization Plan
A Housing Supports Program Pre-Tenancy and Tenancy Sustaining provider who is authorized to deliver services to a member must create a Housing Stabilization Plan following the start of services to support housing search and placement OR immediate stabilization in current housing.
