Federal Health Innovation Grant Launches
CCHP, the Rutgers Center for State Health Policy, and PICO were excited to welcome partners from Neighborhood Health Centers of the Lehigh Valley in Allentown, Pa., Truman Medical Centers in Kansas City, Mo., Multicultural Primary Care Medical Group in San Diego, Calif., and Metro Community Provider Network in Aurora, Co., for a kickoff conference for our CMMI Healthcare Innovations Grant. This conference, held in Philadelphia on October 4 and 5, was a unique opportunity to connect and strategize with others who are working diligently to improve care and change the healthcare system in their communities.

In July 2011, CCHP was one of 107 awardees to receive a Healthcare Innovation Award under the Center for Medicare and Medicaid Innovation. As part of this grant, we’ll be helping to provide technical assistance to the four sites as they begin to build their super utilizer intervention programs. This award also provides funding for CCHP to expand its current outreach services to approximately 1,000 Camden City residents who are frequent users of hospital and emergency department services. Our Link2Care intervention seeks to save $6 million in healthcare costs over three years by reducing hospital re-admissions and increasing access to primary and specialty care care outpatient services.
