Join the movement as we convene the first leadership gathering of a national community working to improve health care delivery for individuals with complex health and social needs.

Our first annual conference, launching the National Center for Complex Health and Social Needs, will take place December 8-9 in Philadelphia, at the Sheraton Society Hill.

The conference will focus on how we can deliver better care through coordinated, data-driven, and patient-centered approaches—with attention to the social correlates of health like housing, transportation, hunger, and education. It’s an opportunity to connect, learn, and share, as we bring together pioneers and new practitioners working around the country, creating together a national hub that will unite and advance a new field of health care.

As the National Center develops, its work must be informed by consumers, patients, and caregivers who can provide valuable insights into how to improve our health and social services systems. We are offering travel and admission stipends to enable patients and caregivers to attend the conference in Philadelphia. Applications are due by September 12; the full request for proposals is available here.
