On Wednesday, March 13, 2013, a Super-utilizer Projects Poster Session will be held during the opening day of the Population Health Colloquium and the Medical Home Summit. These 2 co-located conferences run from March 13 – 15, 2013 at the Philadelphia Marriott Downtown, Philadelphia, PA.

The 5:30 p.m. poster session will be preceded by a full day of presentations, including a half day of pre-conference on various aspects of super-utilizer interventions.   The session will be held in the Exhibit Hall.

Space is limited! Deadline to register for poster presentation is February 26, 2013.

Super Utilizer Clinical Success Story Format

Focus:  Poster presentations should focus on individual patient stories. Please identify one patient who would be described as a success story. Use the following template to guide your summary.

You may create a three page (maximum) summary and later shape it into a series of images suitable for a poster or draft your initial submission via a series of PowerPoint slides.

Be sure to have written patient permission. A copy of the permission does not need to be submitted, but you should have it for your records.

Summary of intervention and outcome

What did your team do to serve this patient?

What impact/results were achieved? Try to use measurable outcomes (clinical measures and/or health service use outcomes)

System-based learning

What was learned about how the systems were failing this person?

What was learned about how to serve others patients like this one?

Space is limited! Deadline to register for poster presentation is February 22, 2013. Once your poster is accepted, additional poster guidelines will be sent to you.
