Meeting patient needs outside of emergency care
The Everyday Pledge aims to reduce the moderately high rates of avoidable emergency department visits among patients whose needs could be better supported by primary care providers in Camden. The program empowers primary care offices to connect patients to vital social services with tools like My Resource Pal, a database that provides up-to-date information on how to access a wide range of local resources. The program also builds understanding of the root causes that lead patients in this population to seek emergency services over their primary care provider.
The Camden Coalition is implementing the Everyday Pledge in collaboration with multiple agencies across Camden. Partnering hospitals include: Cooper University Health Care, Lourdes Health System, Virtua Health, and Jefferson Health’s three New Jersey hospitals. Collaborating providers include: CAMcare, Cooper Family Medicine, Cooper Internal Medicine, Cooper Pediatrics, Fairview Village Family, the Kyle W. Will Family Health Center, Osborn Family Health Center, Project H.O.P.E., and Reliance Medical Group.
