Our diverse partnerships within healthcare and across sectors at the local, state, and national levels are changing the way care is delivered for people with complex health and social needs.
-
Local Initiatives
The Camden Coalition convenes organizations regionally, including hospitals, clinicians, and community-based organizations working together to deliver better care to vulnerable individuals throughout South Jersey. One of our signature products, the Camden Coalition Health Information Exchange, advances robust data sharing among providers in the region.
-
State Initiatives
Our state-level advocacy is informed by our patients’ needs and by insights from our regional partners. We convene healthcare and social services providers, managed care organizations, health systems, payers, and healthcare consumers to advocate for policy change and better-coordinated care.
-
National Initiatives
Our National Center for Complex Health and Social Needs exists to inspire people to join the complex care community, connect complex care practitioners, and support the field with tools and resources that move the field forward. Each year, the National Center hosts Putting Care at the Center, the only annual complex care conference in America.
